Meanwhile, H&K used much of the technology developed for the Mk23 to create the H&K USP, a smaller version of the pistol originally made for the new .40 S&V cartridge. But shortly after its release, the USP was adapted to 9x19mm and .45 ACP calibers. As the Bundeswehr was in desperate need of a new 9x19 service pistol to replace its old stock of Walther P1s, the manual safety version of the 9mm USP was quickly adopted as the P8.

But interest in suppressed weapons remained. The USP .45 Tactical proved suitable for Bundeswehr service. After some minor modifications, it became the P12 of the Bundeswehr. Probably because special operators needed the gun to be ready to fire, the manual safety was removed and replaced with a release lever. Also removed was the over-travel stop on the pistol's trigger, possibly due to operator preference.

The pistols are issued to various special reconnaissance and direct action units in the Bundeswehr. The elite KSK, Fernspacher and other scout troops are known to use the pistol. However, not all scout units use it, lower level formations still use the standard P8.

More special forces get the P12 because it is primarily used as a suppressed pistol, hence the .45 caliber. Since the .45 ACP is a subsonic round in most standard loadings, it can be used in the P12 without acquiring special ammunition that could reduce terminal ballistics.

This is in contrast to the MP5SD, another popular weapon with those units, which actively degasses the 9mm rounds to make them silent, but at the expense of significant velocity from the 9mm rounds, limiting their ultimate effectiveness.
